IF you really want to ALWAYS carry all your gear you are pretty much restricted to a medium to large backpack. There are some shoulder bags that large, but I would find that to be a lot of weight hanging off one side. You might also look at the stuff from http://thinktankphoto.com/. They offer some modular systems, not cheap, but good quality.
Personally I have a backpack which spends 90% of it's life as my gear storage bag. I have a ThinkTank Speed Freak which gets loaded with different lenses depending on what I plan on shooting. For long trips the backpack travels and I bring a small bag that holds my SLR with a lens attached.

A fairly roomy looking bag I was considering myself was the Lowepro Sideline Shooter. This isn't a backpack so might be of interest to the first poster, and also I'd like to know what it's like and don't want to start a new thread. So it's kinda mutually beneficial if you will. Has anyone tried it/do you have any idea how comfortable it might be?

KATA (kata-bags.com) has a bag chooser that is great (as are their bags!)
